What you'll do

• Provide individualized one-on-one assistance under authorized service plans
• assist with daily living activities, personal care, meals, and household tasks
• support transportation, appointments, and community participation
• promote independence, dignity, and personal choice
• monitor safety and report concerns
• maintain service documentation and communicate with families and support teams.
