What you'll do

• Mastery of the Core Model as a Primary Care Physician Leader-in-training.
• Focus on clinical & operational requirements, followed by leadership development programs.
• Takes full accountability for patient care and outcomes.
• Engages with hospitalists for patient care.
• Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
• Leads care team.
• Engages with transitional care team for patients unable to come to the office.
• Supervises and collaborates with Advanced Practice Practitioners.
• Plays an active role in center management and provider development.
• Engages in facilitated practice and leadership rounding.
• Assists in selection, training, onboarding, and coaching of new providers.
• Shadows current leadership.
• Attends leadership development programs.
• Leads Super Huddle and Transforming Care Meeting weekly.
• Assists with center clinical leadership needs.
• Assists leadership in identification and action planning for key clinical outcomes.

What we're looking for

Education

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.

Experience

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required. 1-year clinical experience preferred. Prior leadership experience from residency programs such as Chief Resident, geriatric fellowship or equivalent experience for externals.
