What you'll do

• Manage the plan of care from referral through discharge.
• Complete initial nursing assessments and physical assessments and histories.
• Develop and implement care plans with patients and families.
• Coordinate medications, durable medical equipment, nursing supplies, and contracted services.
• Administer prescribed medications and treatments, document responses, communicate adverse reactions, and modify care plans as indicated.
