Pediatric Hospice RN Case Manager

The Pediatric Hospice RN Case Manager role is field-based in Denton, TX, with local travel and a full-time schedule. Compensation is $80,000–$90,000 per year, with mileage reimbursement. The Staff Pad is partnering with a hospice provider serving children and families in the DFW Metroplex. The case manager provides skilled nursing care to pediatric hospice patients and coordinates their care with families, physicians, aides, and community partners. The role supports individualized, dignified care in patients’ homes and participates in an assigned on-call rotation for emergency support.

What you'll do

• Conduct physical, psychosocial, and environmental assessments
• develop, implement, and document individualized care plans
• communicate with patients, families, physicians, and the care team
• provide clinical guidance to hospice aides and LVNs and assist with training as needed
• complete pediatric admissions and prioritize scheduling
• participate in assigned on-call rotations
• maintain accurate documentation and follow infection-control and agency policies
• support quality improvement and professional development.
