The nurse serves as a central clinical contact for field clinicians, patients, families, physicians, and leadership. The role coordinates care, triages calls, follows up on lab results and orders, supports intake and scheduling as needed, advises field staff, and maintains accurate, HIPAA-compliant records.

What you'll do

• Coordinate patient care across disciplines
• triage patient, clinician, and physician calls
• manage lab orders, results, and follow-up
• support care-plan oversight and outstanding orders
• assist with referrals, insurance verification, start-of-care scheduling, and clinician assignments as needed
• advise field staff on documentation, supplies, infection control, medication reconciliation, and patient concerns
• support quality and compliance initiatives
• maintain HIPAA-compliant EMR documentation.
