Hospice Social Worker

Job overview

The Hospice Social Worker role is onsite in the Billings metropolitan area, MT. Pay is $27–$31 per hour with a master’s degree in social work accreditation or $23–$27 per hour with a bachelor’s degree accreditation. The social worker supports Stillwater Hospice’s hospice team and its patient and community services. The role assesses psychosocial needs of patients and families, provides counseling and referrals, and coordinates with the interdisciplinary team. This work helps patients and caregivers address practical, emotional, and end-of-life concerns while supporting individualized care and bereavement services.

What you'll do

• Assess psychosocial status of patients and families or caregivers
• communicate findings to nurses and the interdisciplinary group
• conduct social evaluations and plan interventions
• counsel patients and caregivers and provide crisis intervention when needed
• educate about cultural needs and advance directives
• provide information and referrals
• maintain clinical records and progress notes
• oversee volunteer and bereavement programs
• participate in individualized care planning, team meetings, and quality-improvement activities
• assist with funeral, financial, legal, and health-care decision planning.
