Home Care Concierge

Job overview

The Home Care Concierge role is onsite in Monroe, MI, with a Monday–Friday schedule, 8:30 a.m.–5:00 p.m., for 40 hours per week. Compensation is $15.92–$18.04 per hour. The role is part of Home Care | Huron Valley PACE and supports participants’ safety, independence, and engagement at home. The concierge provides one-on-one companionship and practical daily-living support, including meal preparation, light housekeeping, medication reminders, and transportation. The role coordinates with the care team and assists participants with appointments and meaningful activities to support their well-being.

What you'll do

• Provide one-on-one in-home support, companionship, and respite care
• assist with daily living activities, light housekeeping, laundry, meal preparation, and medication reminders
• engage participants in activities that promote independence and well-being
• provide transportation and accompany participants to medical appointments as needed
• assist with appointment preparation and documentation
• collaborate with the care team
• participate in training and follow safety and care standards.
