Registered Nurse Case Manager – Hospice

The Registered Nurse Case Manager – Hospice role is field-based in Hudson, WI, with a Monday–Friday schedule from 8:30 a.m. to 5:00 p.m.; pay is $75,000–$90,000 per year. Moments Hospice focuses on comfort, dignity, and compassionate care for patients and families at the end of life. The RN case manager leads clinical care for a patient caseload, managing symptoms and coordinating services with families, physicians, and the interdisciplinary team. The role supports patient comfort and helps families understand care and end-of-life needs.

What you'll do

• Complete initial and ongoing assessments of physical, emotional, and psychosocial needs
• develop and update individualized care plans with patients and families
• provide skilled nursing, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as liaison among families, physicians, and the care team
• participate in interdisciplinary group meetings.
