What you'll do

• Conduct comprehensive nutrition assessments for patients with obesity and chronic conditions
• develop and implement personalized nutrition care plans
• provide medical nutrition therapy and ongoing counseling
• support behavior change and lifestyle modification
• collaborate with the multidisciplinary care team
• monitor and document patient progress and outcomes
• educate patients on evidence-based nutrition strategies
• support adherence through motivational interviewing and coaching.
