What you'll do

• Deliver skilled physical therapy to patients based on an individualized Plan of Treatment.
• Evaluate patient needs and develop rehabilitation goals.
• Adjust treatment plans and techniques as needed.
• Collaborate with the interdisciplinary home health team.
• Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
• Document all visits, assessments, and progress notes accurately and in a timely manner.
• Support patients in maximizing their independence and improving quality of life.

What we're looking for

Education

Master's degree in Physical Therapy

Experience

Master’s degree in Physical Therapy from an accredited program. Current PT license in the state of employment (or eligibility to obtain). CPR certification required. Strong communication, assessment, and problem-solving skills. Ability to work independently in a variety of home environments. Valid driver’s license, auto insurance, and reliable transportation.
