Social Services Director

What you'll do

• Complete social histories and psychosocial assessments and relevant portions of the minimum data set
• maintain timely documentation
• support resident self-determination and explain alternatives when services are refused
• participate in interdisciplinary care planning
• provide or arrange therapeutic interventions and resident/family education
• coordinate discharge planning and referrals
• work with the interdisciplinary team and administration to promote resident rights and psychological well-being
• assist with social service program planning and survey preparation.

Why this role

Requires state certification, licensure, or registration; requires a bachelor’s degree in social work or a related human services field and one year of supervised healthcare social work with geriatric individuals. Two years of skilled nursing facility experience is preferred.
