What you'll do

• Coordinate and monitor behavioral, physical health, and social services
• build linkages with natural supports and community resources
• provide recovery- and wellness-directed coaching and counseling in person and by telephone
• provide case coverage when needed
• maintain accurate records
• participate in team and agency meetings.
