What you'll do

• Conduct initial nursing assessments and quarterly supervisory visits for clients receiving personal care
• develop and document plans of care
• provide clinical oversight and supervision related to personal care
• assess client status and identify changes that may require care-plan updates
• review CNA performance
• communicate significant changes or concerns to the care management team
• complete clinical documentation
• help ensure services follow Rhode Island home care requirements and agency policies.
