Seasonal Shelter Case Manager

What you'll do

• Conduct intake assessments of newly admitted seasonal shelter residents
• develop case plans with short- and long-term goals to increase client functioning and support resettlement in permanent housing
• participate in all phases of case management
• meet with individuals weekly to review progress and address concerns
• connect clients with resources and make referrals related to employment, housing, utilities, budgeting, and other needs.
