What you'll do
- Conduct intake and needs assessments
- manage a caseload of up to 20–25 youth
- develop Individual Action Plans
- maintain regular face-to-face contact and home visits
- build relationships with landlords and housing providers
- coordinate health, education, employment, financial, legal, and benefits services
- facilitate workshops
- enter data in HMIS and Salesforce
- maintain case files and submit monthly reports
- attend required meetings and training.
