What you'll do

• Assess participant needs with the interdisciplinary team
• coordinate participant-centered home and personal care services
• work with the therapy team to develop in-home care plans
• support rehabilitation potential and daily living
• help coordinate transitions to and from home after hospitalization or skilled nursing care.
