What you'll do

• Assist, teach, coach, support, and monitor individuals in activities of daily living and skill development
• transport and accompany individuals to medical and other appointments
• complete notes and program-progress documentation
• participate in developing and implementing service plans
• follow agency and program policies
• meet incident-reporting and service-documentation deadlines.
