• Complete initial and comprehensive OASIS assessments
• develop and revise individualized plans of care
• review medications and perform ordered procedures
• report patient status to physicians
• coordinate interdisciplinary services
• educate patients and families
• maintain clinical documentation
• supervise LPNs and home health aides as assigned
• and support discharge planning.
