What you'll do

• Provide Choices/SNAP case management
• complete intake and eligibility determinations
• assess employment and training needs
• enter participant data in required systems
• guide participants in developing individual employment plans
• counsel on job opportunities, training, and services
• make referrals and follow up
• conduct orientations, workshops, and group presentations
• maintain eligibility documentation and case files
• track referrals
• meet program benchmarks
• complete reports and participate in required staff events.
