What you'll do

• Plan, organize, develop, and implement social services for residents
• complete required documentation and admission assessments
• work with the interdisciplinary team on resident care plans
• support residents, families, and staff
• attend weekly care plan and Medicare meetings and monthly quality assurance and safety meetings
• receive and attempt to resolve resident grievances
• coordinate discharge planning and assist with scheduling outside services.
