What you'll do

• Coordinate and oversee patients’ home care and new admissions
• collaborate with care teams on care plans
• educate families about patient condition, needs, and care
• perform physical assessments and check vital signs
• manage medications and catheters
• complete timely, accurate documentation
• supervise home health aides
• communicate patient progress to the clinical manager, physician, and care team.
