Wellness Coordinator – Registered Nurse (Part-Time)

Job overview

The Wellness Coordinator – Registered Nurse works onsite, with the schedule split between Westerly and Middletown, RI. Pay is $32–$37 per hour, plus a bonus structure. Behavioral Health Group provides opioid addiction treatment services. The coordinator leads the Health Home team’s work to connect patients with coordinated, patient-centered care. The role supports enrollment, care planning, service coordination, patient engagement, transitions of care, community referrals and continuous quality improvement. The schedule is 5:00 a.m.–1:00 p.m., three days per week; travel between locations is required.

What you'll do

• Facilitate Health Home enrollment
• monitor and bill service notes using SAMMS
• develop and update patient-centered care plans
• collaborate on discharge planning
• coordinate preventive, health promotion, mental health and substance use services
• connect patients with community resources
• educate and assist patients
• provide advocacy and wellness interventions
• maintain provider agreements
• oversee continuous quality improvement and regulatory compliance.
