What you'll do

• Partner with members and families to identify supports and services
• assist members in finding and maintaining competitive integrated employment
• conduct intake, enrollment, and ongoing assessments
• develop and maintain Person-Centered Plans
• monitor service provision
• conduct face-to-face visits and assess physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs
• apply criteria for authorizing home- and community-based services and confirm services meet member needs
• travel to members' homes in Hamilton County.
