What you'll do

• Screen and assess individuals and families
• develop individualized housing support plans
• provide case management and referrals
• support life skills, job preparation, and financial literacy
• maintain records and progress reports
• liaise between landlords and tenants
• conduct post-housing home visits
• coordinate case conferences and monitor goals.
