What you'll do

• Verify the plan of care with the RN before each visit
• collaborate with the physician and RN and promptly report changes in patient condition
• teach patients and caregivers self-care techniques
• check vital signs
• review medications and complete medication reconciliation
• perform physician-ordered specialized procedures such as Foley catheter changes
• prepare equipment and materials using aseptic technique
• complete documentation accurately and on time.
