The Home Care Physical Therapist- Part Time (Bergen County) is an onsite role based in Hackensack, NJ. The position offers a base salary range of $60,000.00-$75,000.00 per year, and supports the multidisciplinary therapy team at Ignite Rehab (Metro Physical & Aquatic Therapy). This role exists to provide high-quality, in-home physical therapy services to patients. The therapist evaluates patient needs, develops individualized treatment plans, and delivers care that restores function and promotes independence, contributing to the organization's mission of providing exceptional patient-centered care.

What you'll do

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.
