What you'll do

• Conduct individual and group counseling using evidence-based practices
• collaborate with a multidisciplinary team on treatment plans
• assess patient progress and adjust interventions
• develop discharge and continuity-of-care plans
• maintain caseload documentation and updates
• and participate in team meetings and professional development.
