What you'll do

• Manage a client caseload focused on primary care screening and monitoring
• ensure initial and ongoing screenings
• obtain and enter available primary care screenings into the electronic medical record
• provide health education
• make and track referrals
• assist with appointment scheduling
• monitor attendance at external appointments
• coordinate care with providers with client consent
• document education, referrals, and collaboration.
