Description

Complete comprehensive Nutrition Assessments for newly admitted patients within 48-hours of admission. Complete comprehensive Follow up Nutrition Assessments 2x/week. Prescribe appropriate meal plan to support identified nutrition goals in a manner consistent with the philosophy and objectives of Walden Behavioral Care. Work with patients and support persons to promote completion of meal plan.
