What you'll do

• Coordinate prenatal, perinatal, postpartum, newborn, infant, and pediatric care
• track patient engagement and program measures
• coordinate appointments, referrals, admissions, and follow-up
• conduct nurse-led visits within RN scope and approved protocols
• provide patient education
• document encounters and outcomes
• collaborate with medical, behavioral health, traditional healing, and community partners
• support grant reporting and program audits.
