The Rehabilitation Liaison role is an onsite, full-time position in 00935, Puerto Rico. Compensation is not specified. The liaison evaluates admission needs in relation to clinical requirements, therapies, facilities, and medical plans, and works with the multidisciplinary team to coordinate admissions. The position also visits centers across the island to support referral and admission goals.

What you'll do

• Coordinate admissions for the Rehabilitation Center
• represent the institution and its services to hospitals, medical offices, and elder-care centers
• seek and respond to rehabilitation patient referrals
• evaluate admission needs according to clinical needs, therapy requirements, facilities, and medical plans
• work toward referral and admission goals
• educate patients and the community about services and treatments
• communicate with the multidisciplinary team
• visit centers across the island.

Requires a bachelor’s degree in a health science field, a current professional license, CPR, health and conduct certificates, specified vaccination certificates, a personal vehicle, and a driver’s license. Requires two years of health-field experience and two years of service-sales experience; bilingual verbal and written communication skills are requested.
