What you'll do

• Complete patient databases, treatment plans, progress notes, and discharge summaries
• orient patients to program requirements and rights
• assess and place patients according to treatment phases
• counsel patients in crisis or regarding program compliance
• arrange community referrals
• conduct treatment plan reviews
• participate in case conferences
• facilitate group counseling and intakes
• collect and test urine/swab drug screens
• submit reports and attend supervision and team meetings.
