Intake Nurse

The Intake Nurse role is remote, with full-time hours Monday through Friday, 8:30 a.m.–5:00 p.m. local time. Compensation is not specified. The role helps members understand the program and device use while assisting the Provider Team with assessment and enrollment.

What you'll do

• Welcome and inform new members during enrollment
• accurately collect medication lists, medical history, and other pertinent medical information
• assist the Provider Team with member assessment and enrollment
• educate members about program details and benefits and device-use expectations
• order remote monitoring devices
• conduct high-quality, high-volume calls
• contribute to a kind, collaborative, and respectful team culture.

Why this role

Requires an active, unrestricted LPN multistate compact license in a nursing compact state and at least two years of hands-on nursing experience. Telehealth and chronic care management experience are strongly preferred; English and Spanish fluency is highly preferred.
