What you'll do

• Assess psychosocial status of patients and families/caregivers and communicate findings to the interdisciplinary group
• conduct social evaluations and plan interventions
• counsel patients and families and provide crisis intervention when needed
• educate about cultural needs and advance directives
• participate in individualized care planning and team meetings
• assist with funeral, financial, legal, and healthcare decision planning
• maintain clinical notes and communicate changes to physicians
• coordinate with community health contacts and agencies
• provide information and referrals
• participate in on-call duties
• support continuity of care.
