Bilingual Family Coach / Case Manager (Spanish)

What you'll do

• Interview clients and families and assess needs

• complete enrollment and assessment paperwork
• maintain service plans, case notes, financial distributions, and referrals
• coordinate therapy and crisis intervention as appropriate
• connect participants with community resources
• organize and facilitate meetings, trainings, and support groups.
