Job overview

The Registered Nurse (RN) — Home Health role is onsite in Cleveland, OH, with compensation of $65–$75 per hour. The nurse provides intermittent skilled care to patients and clients in their residences under physician orders and the direction of nursing leadership. The RN evaluates and addresses individual nursing needs, delivers care under established plans, and coordinates with patients, responsible parties, physicians, and agency personnel. Documentation and follow-through support continuity and quality of home-based care.

What you'll do

• Provide services according to the plan of care
• conduct initial evaluations and regular reassessments
• initiate and revise care plans
• provide services requiring specialized nursing skills and preventive or rehabilitative procedures
• prepare clinical notes and care-conference summaries
• coordinate services and communicate changes in patient condition
• counsel patients and representatives
• participate in in-service programs, peer review, and quality improvement as assigned
• process orders and complete certification, recertification, and discharge documentation
• take assigned on-call duty nights, weekends, and holidays.
