What you'll do

• Complete skilled nursing assessments in patients’ homes
• develop, update, and follow individualized plans of care
• provide visits according to physician orders and home health regulations
• administer ordered medications and treatments
• monitor vital signs and patient condition
• provide wound care, dressing changes, education, and instruction
• perform venipuncture, lab draws, and IV-related services as needed and within scope
• complete OASIS assessments and documentation
• coordinate care and communicate patient changes to the team.
