What you'll do

• Plan, organize, and direct hospice care services, including initial evaluation, assessment, pain and medication management, and care-plan development
• revise care plans as patient needs change
• prepare timely clinical notes
• coordinate care with an interdisciplinary team and primary care physicians
• coach and educate patients and caregivers.
