What you'll do
- Assist the Social Service Director, consultant, or administrator with planning, developing, implementing, evaluating, and directing facility social services programs
- meet with administration and clinical staff as directed
- participate in discharge planning, care plans, and resident assessments
- encourage resident and responsible-party participation in care planning
- assist with transportation to medical appointments and discharge
- document social service interactions in medical records
- support policy development and annual review
- promote resident rights and dignity
- assist with advance care planning and review of advance directives
- report suspected abuse, neglect, or exploitation and assist with investigations as assigned
- help residents access treatment and resources for mental illness or psychosocial difficulties.
