What you'll do

• Identify and counsel psychosocial issues
• educate patients and families
• complete initial psychosocial assessments and KDQOL
• participate in interdisciplinary assessment and care planning
• identify social agencies and resources
• provide information and referrals
• advocate for patients
• participate in QAPI activities and support groups
• educate staff on psychosocial care.
