Through education, care coordination, advance care planning, and connections to healthcare and community resources, the role supports informed decisions and continuity of patient-centered care.
What you'll do
- Conduct face-to-face patient visits
- build relationships with patients and families
- initiate goals-of-care conversations
- assess needs and identify healthcare, home care, and community resources
- educate about disease progression, treatment options, palliative care, and services
- assist with advance care planning and disease-specific directives
- coordinate with providers and community resources
- communicate care plans and changes to the clinical team
- document visits accurately.
