Hospice RN Case Manager - Full-Time

The Hospice RN Case Manager role is an onsite position based in Goose Creek, SC, with a base pay range of $83,000 to $90,000 per year. This role supports the hospice care services at Interim HealthCare, contributing to the organization's mission of putting patients first. This role exists to provide exceptional hospice care to patients and families. The RN maintains a specified case management load, completes comprehensive assessments, and coordinates care among the interdisciplinary group to ensure the continuous assessment of patient needs.
