TCM Care Manager Extender

• Conduct outreach, engagement, and follow-up with members

Why this role

Requires lived experience with I/DD or TBI and knowledge of the North Carolina Medicaid system, Certified Peer Support Specialist status with behavioral health lived experience, and parent/guardian experience with at least two years of direct care (not for one’s own child). Spanish proficiency is preferred.
