The Wound Care Nurse Practitioner — Home Visits role is an onsite, field-based position serving Brooklyn, Queens, and occasionally Manhattan, with the listing based in Jamaica, NY. Pay is $70–$80 per hour. 180 Degrees Medical provides wound care and primary care services in patients’ homes and community settings. The nurse practitioner independently evaluates, diagnoses, and manages acute and chronic wounds, coordinating treatment with care teams and educating patients and caregivers. The work supports patient-centered wound management through clinical assessment, treatment planning, documentation, and follow-up coordination.

What you'll do

• Perform comprehensive wound assessments and develop treatment plans
• diagnose and manage acute and chronic wounds
• order and interpret laboratory studies, cultures, imaging, and vascular testing as appropriate
• prescribe medications, wound care supplies, and advanced therapies
• document wound measurements and photographs when appropriate
• coordinate care with providers, agencies, facilities, and families
• educate patients and caregivers
• maintain electronic medical records
• participate in interdisciplinary case discussions as needed.

Why this role
