What you'll do
- Assess residents at program entry and annually
- identify health risks and address medical or psychiatric concerns
- make referrals
- respond to emergencies and follow up
- participate in Individual Support Plans
- document symptoms, encounters, and progress
- interpret diagnostic tests and share findings
- coordinate with behavioral health and healthcare providers
- monitor care, diet, and activity
- support medication preparation and education
- facilitate resident health education groups
- help develop health improvement programs
- maintain nursing supplies.
