Permanent Supportive Housing Case Manager

Job overview

The Permanent Supportive Housing Case Manager role is onsite in Fall River, MA. Compensation is $21 per hour. Catholic Charities’ Permanent Supportive Housing program serves individuals and families who have experienced homelessness. The case manager assesses client needs, develops client-driven plans, and connects people with community resources to address barriers to housing stability. The role works with the Program Coordinator and other staff to support a safe environment and long-term housing stability.

What you'll do

• Complete initial needs assessments
• develop short- and long-term client-driven case plans
• connect clients to employment, public assistance, education, mental health, clinical, and substance-use resources
• meet with clients weekly
• conduct biweekly unit inspections
• maintain case files and documentation
• participate in an on-call rotation and respond to emergencies.
