Hospice Registered Nurse – Primary RN

Job overview

The Hospice Registered Nurse – Primary RN role is based in Grand Junction, CO, with care provided wherever patients call home. The schedule is Monday–Friday, 8:00 a.m.–5:00 p.m., with occasional on-call and weekend requirements. Pay is $38.02–$43.05 per hour depending on degree level: ASN pay is $38.02–$41.75 per hour and BSN pay is $39.20–$43.05 per hour. HopeWest describes its work as changing experiences of aging, illness, and grief for families. The Primary RN manages an assigned patient caseload and leads the interdisciplinary team. The nurse assesses symptoms, supports patients in identifying care goals, and helps caregivers build confidence in caring for family members.

What you'll do

• Provide hospice care in patients’ homes, nursing homes, group homes, or assisted living facilities
• assess patients and manage symptoms
• help patients identify personal goals of care
• teach and support caregivers
• lead the interdisciplinary team for an assigned caseload
• communicate with patients and families.
