Community Health Worker

Job overview

The Community Health Worker role is onsite in Washington, DC, in an office-based position that also includes assigned home and community visits. Compensation is $22–$27 per hour. The worker supports members as a liaison among the healthcare system and community resources, collaborating with the Case Management team. The role builds member engagement and helps reduce barriers to care through outreach, culturally responsive education, advocacy, and care navigation. It supports access to healthcare and community services while reinforcing care plans and referring clinical questions to licensed professionals.

What you'll do

• Conduct telephone outreach and assigned home or community visits
• build culturally responsive relationships with members
• provide health education and advocacy
• collaborate with Case Managers on member engagement
• help members navigate healthcare and community resources
• assist with appointment scheduling, transportation coordination, and follow-up
• reinforce care plans and refer clinical concerns to licensed professionals
• participate in outreach events and health fairs
• document interactions, referrals, and activities accurately and promptly.

Why this role

High school diploma or GED required; meaningful experience with community organizations and a valid driver’s license with a clean driving record required. The role does not perform clinical assessments, independently develop clinical care plans, or make clinical decisions.
