Description

Demonstrates accountability and oversight for outcomes, strong clinical care, and cost-effectiveness for each patient. Acts as a health coach, helping patients set and achieve health goals. Leads center Super Huddle and Transforming Care Meeting weekly. Partners with Center Manager/Center General Manager to co-lead center operations.
