Community Relations Representative — Hospice and Palliative Care

What you'll do

• Develop relationships with hospitals, physicians, discharge planners, senior living communities, and other referral partners
• identify business opportunities and expand the referral network
• conduct community outreach and establish trusted connections
• educate healthcare professionals and families about hospice and palliative care
• generate referrals through outreach, relationship development, and follow-up
• collaborate with clinical and admissions teams
• track outreach activities, referral trends, and business development results.
